Revision Date January 1 2020 Background The Child Health and Disability Prevention CHDP program is a State program of health screening for children in California which incorporates the federally mandated Early and Periodic Screening Diagnosis and Treatment EPSDT program CHDP is available to all MediCal beneficiaries under the age of 21 This section provides an explanation of the purpose and provisions of CHDP This section is updated to the current format and updates procedures Purpose The purpose of screening is the earliest possible detection correction and prevention of problems in health and development which are potentially handicapping or could lead to more serious medical problems Policy A Provisions Health screening is a series of evaluations by physicians or paramedical staff such as pediatric nurses or nurse practitioners supervised by a physician The screening evaluation includes health and development history physical examinations developmental assessment vision and hearing tests dental inspection lab tests appropriate for age and population groups immunizations nutritional assessment and other tests as needed Also children 3 years of age and older can be referred to a dentist for a yearly checkup preventive dental care and needed treatment Under EPSDT the above tests are done at periodic intervals according to the age of the child The screener must explain the results of the screening to the parentcaretaker of the child and make a referral for diagnosis and treatment to the appropriate source of medical care when necessary B Availability Lists of CHDP providers of medical and dental screenings by geographical area are available to each FRC and on the internet The lists include physicians and dentists who provide CHDP services as well as the local health department Contracted Health Care Plan HCP providers must provide CHDP services to all eligible children who are enrolled in the HCP Procedure See above Program Impacts None References ACWDL 8130 Sunset Date This policy will be reviewed for continuance by December 31 2022

Revision Date January 1 2020 Background State rules require that all MediCal applicants who are under 21 years of age or are responsible for children under 21 years of age and all eligible pregnant women be informed of CHDP services Workers must Inform all eligible MediCal applicantsbeneficiaries of CHDP services Make referrals to CHDP Document the informingreferral procedures Purpose This section is updated to the current format and updates procedures Policy A Application The DHS PHE265CHDP must be included in the Supplemental Application Packet when the application includes children under the age of 21 B Intake Ensure that the applicant has been informed of CHDP benefits and make referrals to CHDP for all eligible people who request CHDP services C Intake Referrals Enter a Y or N in the mandatory fields on the Collect CHDPSocial Services Request window CHDP tab Enter a Y in one of the following fields to make a referral for CHDP services Regular checkups and Dental Services through CHDP Medical Services only through CHDP Dental Services only through CHDP Assistance with Doctors Appointment for Checkup or Assistance with Transportation D Documentation The entries above document that the CHDP information was provided to the applicantbeneficiary E Continuing The CHDP informational brochure is included in all MediCal and CalWORKs redetermination packets Follow the actions in Procedure below Procedure Step Action 1 Explain the information contained in the brochure as part of the redetermination process 2 Complete the CHDPSocial Services Request window CHDP tab to document that the beneficiary received the required information and was either referred to or rejected CHDP services
