902502 GR EMPLOYABILITY EVALUATION GREE A This section provides information on the GREE process for the GR General Program B The GREE provides clients who cannot obtain medical verification with Policy the means of obtaining verification of employability status at no cost to the client Referrals to these providers are for the sole purpose of employment evaluations based on a specific medical complaint by the applicantrecipient A GREE appointment is not considered ongoing treatment C Applicantsrecipients who choose to have the County pay for the Appointments employability evaluation are given an appointment at the appropriate GREE clinic Appendix C using the GREE Referral form 1140 HHSA Appendix D The original 1140 HHSA is given to the client and the copy is imaged in the case Note GREE appointments are scheduled according to zip code Homeless clients are scheduled using the district office zip code The applicantsrecipients identifying information and the patients statement of medical employability as it appears on the 1165 HHSA Appendix E shall be entered on the GREE appointment schedule 1166 HHSA Appendix F The GREE appointment schedule must be faxed to the GREE clinic at least 24 hours prior to the scheduled appointment The GREE appointment schedule must be maintained in the FRC for 4 months D The table below shows how to process the results of the GREE Employability appointment Evaluation If the 1145G indicates the applicantrecipient Then the applicantrecipient will be Can do the Work Project or Can do Light Duty Work Project aided as AB if otherwise eligible and subject to job search and work project requirements See chapter 90700 Continued on next page 902502 GR EMPLOYABILITY EVALUATION GREE Continued EmployabilityEvaluation continued E Disagreementwith EmployabilityEvaluation If the 1145G indicates the applicantrecipient Then the applicantrecipientwill be Cannot do the Work Project evaluated to determine if eligible under UE or IAP components See chapter 90700 and 90800 If the GREE indicates that the client is able to perform the work project and the applicantrecipient disagrees with the evaluation the recipient may provide a second opinion from a private provider A second GREE appointment will not be scheduled The worker shall give the client a CSF 24 or 1145 HHSA marked Second Opinion in the upper right hand corner to take to the private provider Note CMS will not pay for the second opinion Workers must inform applicantsrecipients who disagree with the GREE and have CMS coverage that CMS will not pay for this evaluation GRPG Letter 69 212
