63-356.1Applicant/Recipient Request for Information/Photocopies |
Information relating to eligibility that has been provided solely by the applicant/recipient or authorized by the applicant/recipient and is contained in applications or other documents completed by the applicant/recipient and kept by HHSA may be released to the applicant/recipient or his/her authorized representative. Examples include: · Bank verification requests. · Employment verification requests. · Medical verification requests, etc. Staff may photocopy specific items in the case record for the applicant/recipient and/or his/her authorized representative. Note: Photocopies, in response to general requests (the whole case file, all SAR 7’s, etc.) will not be provided. Also, photocopies of case narratives will not be provided. |
63-356.2Information Provided by a Third Party |
Information provided confidentially to the Agency by a third party and not provided solely by the applicant/recipient will not be disclosed to the applicant/recipient or his/her authorized representative. Examples include: · PAFD reports; · Informant identity; and · Other privileged communication. |
63-356.3IEVS Information |
Case information obtained through the Income Eligibility Verification
System (IEVS) requires special handling to protect the confidentiality of
IEVS information that may have been mismatched to the applicant/recipient or other
household members. Workers are to use the following guidelines concerning the
sharing of IEVS information with the applicant/recipient or his/her
authorized representative: ·
If
the IEVS report verifies information previously reported by the
applicant/recipient, the report will be treated as verification authorized by
the applicant/recipient and may be released to the applicant/recipient. ·
If
the IEVS report does not appear to pertain to the applicant/recipient, or if
it contains previously unreported information, it will be treated as
information provided confidentially by a third party and the report may not
be released until the discrepancy has been clarified and the accuracy confirmed. ·
Sufficient
information from the IEVS report may be provided to the applicant/recipient
to allow them to obtain additional verification and/or clarification. Example: EDD reports a match of the SSN to a UIB recipient. The name does not match and the recipient claims no UIB income. The EDD Field Office number from the report may be provided to the recipient to aid EDD in clarifying the discrepancy. The name of the UIB recipient may not be released. |
