Revision Date October 1 2020 Background The Wrap Around program consists of Intensive Case Management services in conjunction with Senior Mentoring It provides closely supervised intensive case management assistance to Employment Services participants from a team of professionals Purpose The purpose of this revision is to change the length of services period from 90 days to 180 days The purpose of the Wrap Around program is to remove barriers that are preventing the customers from participation in Employment Services ES activities by providing intensive and supportive relationships between the family Employment Case Manager ECM and other support systems such as schools and employers Because the entire family is mentored participants learn how to develop contingency plans for working parent issues The health and wellbeing of the child and the entire family is considered to make the transition to work a positive experience for all family members PolicyTarget Populations Currently sanctioned from ES within the last six months Individuals returning to ES activities from Behavioral Health Services treatment CalLearn families transitioning to ES CalWORKs families who have known barriers to selfsufficiency Other populations identified by the County Program Services Identification and recruitment of families to receive services Family assessment to identify barriers to selfsufficiency Using plan of action to remove barriers to selfsufficiency Coordination of all resources needed to achieve plan of action Retention services to follow up with participants six months after leaving the Intensive Case Management program Length of Services Services are to be provided for a period of 180 days to each family Exceptions to continue working with a family beyond 180 days may be requested Partnerships ES sites will have documented collaborative agreements with organizations offering resources that may aid in the removal of barriers and support selfsufficiency for families Procedure None
